Narrative

RESIDENTIAL SERVICES provide a broad menu of living options and supports. Supports are determined by each person's personal preference and the level of care that is needed.

Our services include Community Residences, Independent Living, Adult Foster Care and Shared Living.

What we offer:

  • Community Residences house people who need 24 hour support. Services are tailored to each person's needs. People can increase their potential for independence and provide meaningful community connections. Services include:
    • intake and needs assessment
    • transition planning
    • individualized service plans (ISPs)
    • skills development and training
    • case management
    • recreation 
    • community access
  • Independent Living helps those who have mastered many independent living skills. They still need more support. Services include
    • case management and support
    • help accessing community services
    • training
    • networking 
    • recreation
  • Adult Foster Care provides clinical and financial help to qualifying families and caregivers. We work to keep their loved ones at home and out of residential institutions. Eligible people must be at least 16 years of age, receive MassHealth Insurance and require daily help with ADLs like:
  • mobility (walking, sitting and standing)
    • bathing
    • dressing
    • toileting
    • eating 
    • transferring 

Caregivers of people who are accepted into this program receive payment based on the person's level of need.

  • Shared Living provides people the opportunity to become part of a family and a community. Shared living providers are carefully picked. They are trained to meet the needs and wishes of people. Ongoing monitoring and evals include monthly visits by the shared living case manager. We give access to the behavioral clinician, access to the residential nurse and crisis and safety management.
