Options for Community Living

UCP’s Options for Community Living program offers individualized, community-based supports for adults with physical, cognitive, or developmental disabilities. Program is for people who live in accessible apartments across eastern Massachusetts. This program promotes independent living. We have flexible assistance based on each person’s needs and goals.

Members receive support in key areas such as:

  • Personal care attendant (PCA) management
  • Transportation and money management
  • Health and household management
  • Self-advocacy and decision-making
  • Social and recreational activities

Our goal is to empower people to live as independently as possible. Supports are tailored through goal setting and annual assessments. We have an emphasis on skill-building, self-direction. We gradually reduce help as independence grows. Case managers provide only the level of help needed We encourage members to take the lead in their own lives when they can.
