Narrative
The Transition to Adulthood Program (TAP) supports high school students with disabilities, ages 14-21, as they prepare for adulthood. Using a personalized, peer-based approach, TAP helps students build independent living skills, understand their options, and work toward personal goals.
Members receive support with school and transition planning, self-advocacy, community involvement, and family support.
Through Skills Training, Your Access Advocates utilizes the “Peer Model” approach. Meaning, trained Skills Trainers work with consumers to develop an independent living plan and together, work towards attaining consumer-directed goals. TAP provides support around:
- Individualized Education Plan (IEP) understanding and advocacy
- Individualized Transition Plan (ITP) support
- Support to family
- Community Education and Recreation
